Form for setting up a staff account under the domain polsl.pl

In order to obtain an employee account in the POLSL domain:

1. Fill in the form below and then click "Send".
2. Print the generated application.
3. Complete missing signatures and stamps
4. Deliver the completed version to the IT Centre (by internal post, scan by email to rn4-ci@polsl.pl or fax to 32 237-27-86).
